1、论著中国民族民间医药TreatisChinesejournalofethnomedicineandethnopharmacy·29·金属蛋白酶3、抗环瓜氨酸肽和类风湿因子分型对类风湿关节炎临床检测的应用杨秉潜广东省梅州市人民医院,广东梅州514000【摘要】目的:基质金属蛋白酶一3(MPP一3)、抗环瓜氨酸肽(CCP)抗体、类风湿因子分型(RF)检测对于类风湿关节炎(RA)患者疾病进程的临床预测价值。方法:采用ELISA检测法,测定血清MMP一3、抗CCP抗体、RFlgM、RFlgG、RFlgA的含量。结果:(1)MMP一3、抗
2、CCP抗体及RFlgM、RFlgG、RFlgA在健康对照组均有显著性意义(P<0.01),抗CCP、RF在RA各组血清水平均高于非RA及对照组(P<0.O1);MMP一3在RA组具有较高含量,显著高于非RA组、健康对照(P<0.01);在RA1组与RA2组中水平比较差别明显(P<0.01),在RA2组与RA3组中水平比较差别明显(P<0.01)。(2)RA组中MPP一3、抗CCP抗体、RFlgM+RFlgG+RFlgA的敏感度分别为65.3%,68.7%,71.8%。(3)MMP一3、抗CCP与RFlgM+RFlgG+RFlgA
3、在RA组中特异性分别为70.2%,95.1%,83.1%。(4)五项指标的联合检测敏感度为32.8%,联合特异度为100%。结论:联合检测五项指标可降低RA的漏诊率。动态联合测定抗CCP抗体、RFlgM、RFlsG、RFlgA、MMP一3能更有效地及时监测RA的疾病进展,对指导临床诊治及干预有积极意义。【关键词】类风湿关节炎;基质金属蛋白酶一3;抗环瓜氨酸肽抗体;类风湿因子分型【中图分类号】R593.22【文献标识码】A【文章编号】1007—8517(2013)12—0029—02Abstract:0bjective:Matri
