1、·2022·中华实验外科杂志2014年9月第31卷第9期ChinJExpSurg,September2014,Vo1.31,No.9·临床研究·利用代谢组学方法筛选肝门部胆管癌血浆诊断标志物李之帅谭蔚锋姜小清张柏和罗祥基刘辰冯茂辉【摘要】目的探讨外周血代谢组学对于肝门部胆管癌具有早期诊断价值的分子标志物。方法选择临床确诊为肝门部胆管癌30例、原发性胆管结石15例、胆总管囊肿的患者l5例及健康对照者20例,应用气相色谱一飞行时间质谱联用仪(GC—TOFMS)检测血浆中的小分子代谢物。对检测结果进行多变量统计学分析,建立鉴别诊断模型,
2、筛选出有意义的血浆诊断标志物。结果鉴定了右旋一半乳糖醛酸为1种肝门部胆管癌特异的血浆代谢物。其在肝门部胆管癌组、健康对照组、胆管囊肿组、胆管结石组中的峰面积平均值分别为0.001204584、0.00011087、0、0.0000023893(Ⅱ1Au×min)。其在肝门部胆管癌组中含量显著高于健康对照组(P<0.05)、胆管囊肿组(P<0.05)、胆管结石组(P<0.05)。结论通过对肝门部胆管癌、原发性胆管结石、胆总管囊肿患者和健康人群的血浆代谢组学研究,结合多变量统计分析方法,表明代谢组学技术有助于诊断肝门部胆管癌及研究其发
