1、临床医学论文•后踝骨折CT分型及其对临床的指导意义作者:王志生,王志强,李立东,庞海涛,周瑞红,焦翔【摘要】[目的]对后踝骨折进行CT分型,用于指导临床治疗。[方法]2003年1月〜2006年10月治疗后踝骨折88例,男53例,女35例;年龄18〜75岁,平均42.3岁。所有患者进行X线及CT检查,根据后踝主要骨折块大小、部位和骨折线等进行CT分型,并给予相应治疗。[结果]根据CT图像,后踝骨折分为四型:I型(稳定型)23例,其中I1型(撕脱骨块型)5例(5.7%),12型(小楔形骨块型)18例(20.4%);I
2、I型(临界型)26例,其中III型(中等楔形骨块无距骨后外脱位型)15例(17.1%),112型(中等楔形骨块合并距骨后外脱位型)1]例(12.5%);III型(大楔形骨块不稳定型)18例(20.5%),IV型(屮间贯穿型)21例(23.8%)。所有患者均获得随访,平均随访时间2.6年(1〜4年)。根据BairdJackson评分系统评估疗效:优62例,良13例,可8例,差5例,优良率85.2%。[结论]CT能够明确后踝骨折的组织解剖学变化,对临床治疗冇重要指导意义。【关键词】后踝骨折;分型;X线计算机;体层摄影
3、术Abstract:[Objective]Toclassifyfracturesofposteriormalleolusbycomputedtomographyandguideitsclinicaltreatment・[Method]FromJanuary2003toOctober2006,eighty・eightpatientswhohadbeentreatedsurgicallyfortheposteriormalleolarfractureswerereviewed.Therewere53malesand3
4、5females.Themeanagewas42.3ycars(ranged,18^75years)・Al1thepatients'preoperativeradiographandcomputedtomographicscanswerereviewed,andeachfracturewascategorizedaccordingtothesize,locationandfracturelineofthemajorfragmentandthestablestateofmortise.Theelassificati
5、onwasappliedtotreat88patients・[Result]Onthebasisofthecomputedtomographicimages,theposteriormalleolarfractureswerecategorizedintofourtypes・ThereweretypeI(stable)in23cases,typeII(borderline)in26cases,typeIII(bigunstableposterolateralobiique)in18casesandtypeIV(m
6、edialextension)in21cases.TypeIincludedtypeI1(smal1shel1)in5casesandtypeI2(smal1posterolateralobiique)in18cases.TypeIIincludedtypeIIl(moderateposterolateralobiiquewithoutlateralposteriordislocationoftalus)in15casesandtypeII2(moedrateposterolateralobiiquewithla
7、teralposteriordislocationoftalus)in11cases.Allpatientswerefollowedupforanaverageof31months(rangedfrom12to48months).AccordingtotheBairdJacksonscoringsystem,theresultswereratedasbeingexcellentin62cases,goodin13cases,moderatein8cases,andpoorin5cases,withthegoode
