1、纤维支气管镜引导下鼻与经口气管插管机械通气抢救COPD呼吸衰竭的比较张倩云（梅州市人民医院呼吸内科514000）【摘要】目的：研究经鼻与喉镜下经口气管插管在COPD呼吸衰竭患者中的治疗效果的对比观察。方法：40例COPD呼吸衰竭患者随机分为经鼻气管插管组（20例）、经口气管插管组（20例），分别简称经鼻组和经口组。经鼻组患者经鼻气管插管，经口组患者经口气管插管。观察两组患者治疗前后血气变化、置管时间、鼻窦炎感染情况、口腔护理时间、痰痂形成、拔管成功率、意外拔管发生率等情况。结果：两组患者血气变化、鼻寞炎发生例数、置管时间、拔管成功率无显著性差异，但在留管时间、口

2、腔清洁时间、意外拔管等方面，经鼻气管插管组具有明显的优势。结论：对COPD呼吸衰竭患者实施经鼻气管插管并机械通气的治疗措施。与经口气管插管比较，具有疗效肯定，能延长置管时间，缩短口腔护理时间，有效防止口腔感染，使患者感觉舒适，防止意外拔管的优点，值得在临床上应用和推广。【关键词】气管插管呼吸衰竭鼻窦炎【屮图分类号】R56【文献标识码】A【文章编号】1672-5085（2014）26-0013-02ComparisonoffiberbronchoscopeguidednasalandLaryngoscopicorotrachealIntubationmechani
