1、膜瘤(26.9%),20例为颅咽管瘤(15.3%),海绵状血管瘤4例(3%),神经鞘瘤4例(3%),Rathke's囊肿6例(4.6%),鞍旁囊肿1例(0.8%),畸胎瘤1例(0.8%),转移瘤1例(0.8%),颗粒细胞瘤1例(0.8%),孤立性纤维肿瘤1例(0.8%),颗粒细胞瘤1例(0.8%),黑色素瘤1例(0.8%),生殖细胞瘤1例(0.8%),皮样囊肿1例(0.8%)。结论:①眶上外侧入路能充分显示鞍区解剖结构,并成功应用于鞍区肿瘤的切除。②与传统翼点入路相比,眶上外侧入路开颅时长短,大约5-10分钟;
2、皮肤切口前缘未达耳前颧弓水平,因此降低了对颞肌的损伤;使用了联合的皮肌瓣,避免了对面神经上支的损伤;伤口位于发际线以上,更利于美观;手术仅分离了颞肌的上极和前部,因此降低了术后对咀嚼功能的影响;手术采用小骨瓣从而降低了术后脑脊液漏的风险。手术时间短,降低了麻醉风险,有利于病人术后恢复;住院天数短,减轻了病人的经济负担。③与眶上锁孔入路相比,眶上外侧入路视角及视野更大,对术中出血更容易控制,较眶上锁孔入路更为安全;对手术器械要求低,易临床推广;伤口更隐蔽,更适用于疤痕体质及眉毛稀疏的患者。④在鞍区肿瘤的经颅入路中

6、my.23caseswereexcludedbycoronaryandsubfrontalapproach.130patientsweredividedintotwogroups:thelateralsupraorbitalapproachin44cases,pterionalapproachin86cases.Theclinicalpresentations,neuroradiologicalfindings,microsurgicaltechniques,andoutcomeatdischargeofthe
7、sepatientswereanalyzed.Datawereanalyzedusingt,χ2,ranksumtest.Results:Lateralsupraorbitalapproachgroupof44patientsoperationincisionlengthwas9.5±1.7cm,theaverageoperationtimewas152±41min,anddaysofhospitalizationaverage15.1±3.3.After3monthsfollow-up,40patientso
