1、提要目的:探讨血必净注射液对多器官功能障碍综合征(MODS)患者的炎性介质、血管外肺水和呼吸功能的影响。方法:选择千佛山医院ICU由各种病因导致的MODS患者41例,随机分为对照组(给予常规西医综合治疗.包括控制感染、支持呼吸功能、维持血流动力学稳定、改善微循环、营养支持及其他脏器支持等治疗)与治疗组(常规西医综合治疗的基础上给予血必净注射液100ml加0.9%生理盐水100ml静脉点滴,每次点滴时间在30分钟至40分钟,每天2次,连用7天)。放置PiCCO导管,监测血管外肺水指数(EVLWI)和胸腔
2、内血容量指数(ITBVI)的变化。记录气道峰压(Ppeak)、肺动态顺应性(Cdyn)和血气分析结果。结果:治疗组治疗后7天,监测炎性介质白介素-6(IL-6)、肿瘤坏死因子(TNF-α)明显降低、EVLWI减少,ITBVI趋向稳定;Ppeak和Cdyn改善,氧合指数明显提高。与对照组比较差异均有统计学意义(p均<0.05)。结论:血必净注射液可以降低炎症介质,减轻MODS患者的血管外肺水,改善呼吸功能,提高抢救成功率。关键词血必净注射液;炎症介质;血管外肺水;呼吸功能EffectsofTheXueb
