1、中华现代影像学杂志JournalofChineseModernImaging2007年第4卷第1期·19··论著·颈部淋巴结高频彩超检查及穿刺病理诊断1211尹小梅,戴新,王彦,史凤毅[摘要]目的探讨高频彩超检查在颈部淋巴结定性诊断中的价值。方法对78例彩超引导下细针淋巴结穿刺活检结果进行回顾性分析,比较正常、良性、恶性淋巴结的高频彩超特点,彩超引导下细针穿刺肿大淋巴结的定性诊断。结果正常淋巴结、良性肿大淋巴结和恶性淋巴结肿大的L/S值分别为4154±1169、2104±0175、1171±0168。阻力指数(RI)分别是0157±0110、0163±0111、0176±0117
2、,各组淋巴结的纵横径比值、RI比值差异有显著性(P<0105)。良性肿大淋巴结内髓质增宽、回声增强,皮质变窄,恶性肿大淋巴结内髓质变薄受压偏心,皮质增厚回声低弱不均匀。CDFI显示良性淋巴结内血流丰富、呈树枝状分布规则;而恶性肿大淋巴结内血流呈不规则紊乱的血流信号。结论高频彩超对及时发现颈部肿大的淋巴结,正确区分病变的性质具有重要临床意义,高频彩超引导下颈部肿大的淋巴结穿刺活检是安全、有效的最终确诊手段。[关键词]颈部淋巴结;高频彩超;超声引导穿刺;颈部恶性淋巴结肿大[中图分类号]R73314;R44511[文献标识码]A[文章编号]1681-2824(2007)01-0019-
