1、基于CT图像的轻度胸主动脉钝性创伤的意义探讨朴雪瑞严志强陈英浙江省杭州市儿童医院放射科浙江大学附属第二医院放射科摘要：目的探讨基于CT图像的轻度胸主动脉钝性创伤（BTTAI）的定义和临床意义。方法回归性分析76例BTTAI患者的影像学资料。商定轻度BITAI的定义：1）内膜片，伴或不伴附壁血栓;2）假性动脉瘤，H病变处主动脉直径增加〈主动脉肓径的10%。记录轻度BTTAT患者的治疗方案（手术、支架或药物治疗）、最后一次临床随访结局或复查的MDCT成像状态。结果76例BTTAI患者中有29例（38.

2、2%）患者诊为轻度损伤，其中18例（62.1%）行药物保守治疗，8例（27.9%）接受介入治疗，2例（6.9%）行开放手术治疗，1例（3.4%）在处理主动脉损伤之前死亡。接受MDCT随访的22例患者，14例（63.6%）损伤稳定，4例（18.2%）损伤改善，4例（18.2%）消失。接受药物保守治疗的18例患者有14例进行了MDCT复查，结果未见并发症或损伤进展。结论基于CT的合适的轻度BTTAI定义，可以为合理、安全地保守治疗提供一定的理论依据。关键词：胸主动脉钝性创伤;体层摄影术，X线计算机;治
