1、胰腺间变癌的临床诊治(附10例报道)许雪烽I楼文晖I纪元*邵叶波I施晨晔丨王单松I匡天涛I吴文川I靳大勇丨【摘要】目的了解掌握胰腺间变癌的生物学特征并探讨临床特点、组织学特点、生物学特征、治疗方法及预后。方法回顾性分析我院1999年一2010年间收治的10例胰腺间变癌的临床特点、影像学改变、病理学特征、治疗过程及随访资料结介文献讨论。结果本组胰腺间变癌患者，其中男7例，女3例，平均年龄63.0岁。肿瘤位于胰头5例，胰头颈部1例，胰体尾4例。行胰十二指肠切除术5例，其中2例合并扩大淋巴结清扫术，1
2、例合并门静脉重建，全胰切除1例，行胰体尾和脾切除术4例。病理检查：瘤体平均5.38cmX4.69cmX3.92cm。肿瘤呈浸润生长的特点。5例胰头肿块屮，4例肿瘤侵犯十二指肠、胆总管，侵犯肠系膜上静脉一门静脉1例；1例狀体尾肿瘤侵犯脾动脉。8例患者镜下见肿瘤侵犯神经，其中4例有淋巴结转移。随访9例，失访1例，死亡8例。术后生存8〜20个月，中位生存12个月，均死于肿瘤复发和转移。结论胰腺间变癌是导管腺癌的一个特殊类型,是胰腺高度恶性肿瘤，对传统放化疗可能都不敏感，其牛物学特征和新的治疗方案有待进
