1、31例原发性肝细胞癌患者超声诊断中各项特征研究【摘要】目的对超声检查中原发性肝细胞癌(HCC)的各项特征进行分析,旨在提高原发性肝癌的早期确诊率。方法选择本院经术后病理证实为HCC的31例患者(A组)和非肝内占位病变的96例患者(B组),对两组患者行彩色多普勒超声及超声造影检查,并对超声诊断中的各项特征采取单因素logistic回归分析,并对具有意义的各项检查特征采取多因素logistic回归分析。结果在单因素logistic回归分析中,病灶范围、病灶边界、病灶数量、病灶后方回声、病灶内部回声及病灶是否存在晕环征、彩色多普勒血流信号分级、能量多普勒
2、、脉冲多普勒频谱、造影剂增强方式、占位病变的造影时间-强度曲线中的峰值强度及肿瘤内彩色血管平均密度等超声特征对HCC的预测存在一定意义,而针对上述超声诊断特征中的多因素logistic回归分析中,晕环征、能量多普勒、造影剂增强方式及肿瘤内彩色血管平均密度因素为HCC发生的独立危险因素。结论在HCC的超声诊断中,晕环征、能量多普勒、造影剂增强方式及肿瘤内彩色血管平均密度具有一定诊断价值,通过logistic回归模型可与其他非HCC肝占位病变进行鉴别诊断。【关键词】晕环征;能量多普勒;造影剂增强方式;肿瘤内彩色血管平均密度;原发性肝细胞癌;超声造影;彩
