1、摘要摘要目的:观察L-NAME对严重烫伤早期大鼠血脑屏障通透性和MMP.9表达的影响,探讨MMP.9与严重烫伤引起的血脑屏障损伤之间的关系。方法:建立30%TBSAIII度严重烫伤大鼠模型。健康成年SpragueDawlay大鼠,随机分为四组:正常对照组(A);假烫伤组(B);烫伤组(C);烫伤+L.NAME治疗组(D)。C、D组又设严重烫伤后3、6、12、24、48h5个时相点,用透射电镜观察血脑屏障超微结构变化;紫外分光光度计检测脑组织伊文氏蓝含量;干湿重法测脑含水量;免疫组化染色观察脑MMP.9表达。结果:严重烫伤后大鼠脑组织伊文氏蓝含量、脑含水量明显增高,MMP.9表达升高,差别具有显
2、著性(P<0.01)。电镜显示:烫伤后大脑皮质的神经细胞线粒体肿胀或溶解,突触小泡减少,突触间隙增宽。脑毛细血管周围出现空泡化,内皮肿胀,基底膜厚薄不均。用一氧化氮合酶抑制剂L.NAME治疗后,脑组织伊文氏蓝含量及脑含水量与假烫伤组比较明显下降,经L.NAME治疗后MMP.9表达恢复至正常水平,脑毛细血管、神经细胞、突触及基底膜等超微结构基本恢复正常。结论:严重烫伤后大鼠血脑屏障通透性增高与脑内MMP.9表达升高有关,提示MMP·9是严重烫伤后血脑屏障损伤的关键因素。早期应用L-NAME能明显减轻烫伤对BBB的损伤,可能与L.NAME抑制MMP.9表达有关。关键词:烫伤:血脑屏障;L.硝基.精
