1、脉冲多普勒Tei指数在评价肺动脉高压患者右心室功能中的价值岳湘竹摘要目的本文旨在探讨Tei指数在评价肺动脉高压患者右心室功能中的价值及其影响因素。方法肺动脉高压患者45例，分为两组，一组为肺源性肺动脉高压患者(25例)，一组为继发于房间隔缺损的肺动脉高压患者（20例），正常对照组26例，分别测量相关时间间期并计算右室Tei指数，记录所有观察对象的心率(HR)、右室舒张末期前后径，肺动脉高压患者测其肺动脉收缩压。对数值进行比较分析。结果（1）与正常对照组相比，肺源性肺动脉高压组心率明显加快，IRT、ICT、IR
2、T/ET、ICT/ET均明显增高，ET明显缩短，Tei指数明显增高；（2）ASD性肺动脉高压组心率增加，IRT、IRT/ET、ICT/ET均增高，ICT无统计学区别，ET明显缩短，Tei指数明显增高；（3）正常对照组右室Tei指数与年龄呈正相关(r=0.4,p<0.01)，各病例组右室Tei指数与年龄间无相关性；（4）正常对照组及各病例组右室Tei指数均与心率、右室舒张末期前后径无相关性；（5）肺源性肺动脉高压组右室Tei指数与肺动脉收缩压无相关性，ASD性肺动脉高压患者Tei指数与肺动脉收缩压存在相关关系(
3、r=0.48,P<0.05)。结论(1)Tei指数是评价肺动脉高压患者右室功能简便而准确的多普勒超声敏感指标，且不受心率、右室几何形态影响；(2)右心室功能受损状态受压力负荷增加影响大，受容量负荷增加影响小，但就非高动力性肺动脉高压患者而言，其右室功能受损程度并不与肺动脉收缩压升高程度呈相关关系。关键词超声心动图；肺动脉高压；心室功能；右；Tei指数UsefulnessofTeiIndexforAssessingRightVentricularFunctioninPulmonaryHypertensionYU
