1、加强口腔护理和改良吸痰法对预防呼吸机相关性肺炎的效果比较刘旭荣[摘要]目的探讨机械通气过程屮,强化门咽部护理措施和采取不同的吸痰方法对预防呼吸机相关性肺炎(VAP)的影响。方法选择2008年5月-2011年3月因意外伤害造成胸部外伤、颅脑损伤的患者60例,均建立人工机械通气48h以上,其屮经口气管插管患者20例,气管切幵患者40例,通过历史性分析,随机分为治疗组40例和对照组20例。治疗组门胶护理应用强化门咽部淸洁措施,吸痰时插管病人先采取声门卜吸痰,然后再更换吸痰管,行气管插管内吸痰;切开病人采
2、取从气道上方先吸引,再边插入边吸引,达到气道深部后再缓慢边提起边吸引的方法。对照组门腔护理按常规门腔清洁,吸痰时采取以往一插到底先吸气管内,后行门腔内吸痰的方法。观察对预防呼吸机相关性肺炎(VAP)的作用。结果两组在VAP发生的时间、发生率以及预后情况均有明显差异。结论应用强化门咽部淸洁措施和不同的吸痰方法,能有效预防VAP的发生。[关键词]机械通气强化口咽部清洁不同吸痰法VAP预防作用DOI:10.3760/cma.j.issn.1672-7088.2014.21.63作者单位:741000,甘
