1、RadiologicDiagnosisofOsteoidOsteomaOsteoidosteomaisabenignbonetumorthatoccursmostfrequentlyinmenandboysbetween7and25yearsold.Mostpatientsexperiencepainthatworsensatnightandispromptlyrelievedbytheadministrationofsalicylates。Typicalradiographicfindingsofosteoidosteomaincludeanintraco
2、rticalnidus,whichmaydisplayavariableamountofmineralization,accompaniedbycorticalthickeningandreactivesclerosisinalongboneshaft.AtCT,thenidusiswelldefinedandroundorovalwithlowattenuation(Fig2).Anareaofhighattenuationmaybeseencentrally,afindingthatrepresentsmineralizedosteoid(Fig3).M
3、Rimagingdepictsnotonlythenidusandaccompanyingsclerosisbutalsoadjacentbonemarrowandarticularabnormalities。ThenidushaslowtointermediatesignalintensityonT1-weightedimagesandvariablesignalintensityonT2-weightedimages,dependingontheamountofmineralizationpresentinthecenterofthenidus.Edem
4、ainadjacentbonemarrowandsofttissueandjointeffusionalsomaybeseen(Fig3)。Severalstudieshaveshownthat,comparedwithCT,MRimagingisoflimitedvalueindepictingthenidus。MRimagingsometimesfailstodepictsmallnidi,becausethesignalinthenidusoftenissimilartothatincorticalbone(Fig4).IntraarticularOs
5、teoidOsteomaIntraarticularosteoidosteoma,whichoccurswithinornearajoint,isconsideredaseparateclinicalentity。Themostcommonlyinvolvedjointisthehip.Theankle,elbow,wrist,andkneearelesscommonlyaffected。Imagingfindingsofintraarticularosteoidosteomadifferfromthoseofintracorticalosteoidoste
6、oma.Withintraarticularosteoidosteoma,reactivecorticalthickeningisminimalorabsent,afindingbelievedtobeduetoalackofcambium,theinnerlayeroftheperiosteum.UncommonLocationsofOsteoidOsteomaOsteoidosteomaismostcommoninthefemurandtibia;morethan50%ofcasesoccurintheselocations.Approximately3
7、0%ofosteoidosteomasoccurinthespine,hands,orfeet。Theleastcommonlocationsaretheskull,scapula,ribs,pelvis,mandible,andpatella(Fig7)。Themostcommonlocationofspinalosteoidosteomaisthelumbarsegment(Fig8),followedbythecervicalsegmentandthethoracicsegment.Thesacrumistheleastcommonlyaffected
