1、慢性阻塞性肺疾病并呼吸衰竭机械通气治疗中死亡10例临床分析【关键词】慢性阻塞性肺疾病；呼吸衰竭；机械通气；预后　　慢性阻塞性肺疾病并呼吸衰竭机械通气治疗中死亡10例临床分析(pdf)　　［摘要］目的探讨慢性阻塞性肺疾病(COPD)并呼吸衰竭患者机械通气治疗中死亡的高危因素。方法分析20例COPD并呼吸衰竭机械通气患者的临床资料，并对其中10例死亡患者进行相关因素分析。结果死亡组与缓解组患者比较，死亡组电解质紊乱、低蛋白血症、合并肺性脑病比例明显增高，差异有非常显著性(P<0.01)，而肾功能不全、肝功能不全、合

2、并肺心病比例与缓解组比较，差异无显著性(P>0.05)；机械通气前pH<7.20、PaO2<5.33kPa、PaCO2>10.67kPa比例及机械通气过程中耐药痰菌及呼吸机相关性肺炎(VAP)发生率、机械通气时间与缓解组比较亦明显增高，差异均有非常显著性(P<0.01)。结论正确选择机械通气时机，加强呼吸道管理，合理选择抗生素，积极控制感染，缩短有创通气时间，营养支持治疗对COPD并呼吸衰竭行机械通气患者的预后有重要意义。　　［关键词］慢性阻塞性肺疾病；呼吸衰竭；机械通气；预后　　Cli
