1、·268·医学研究生学报2014年3月第27卷第3期JMedPostgra,Vo1.27,No.3,March,2014论著(临床研究)持续输注右美托咪啶对丙泊酚复合瑞芬太尼静脉麻醉用药量的影响范国祥,张卉颖,耿明倩,陆丽【摘要】目的丙泊酚复合瑞芬太尼静脉麻醉用于长时间手术时费用较高。文中观察术中持续使用右美托咪啶对丙泊酚复合瑞芬太尼静脉麻醉用药量的影响。方法42例接受(微创)腹腔镜手术的患者分成对照组、右美托米啶组。每组21例,术中采用丙泊酚复合瑞芬太尼静脉维持麻醉。对照组未使用右美托米啶;右美托米啶组麻醉诱导前开始静脉持续输
2、注右美托咪啶0.6txg/(kg·h)并维持至手术结束。手术中根据脑电双频指数(bispeetralindex,BIS)监测与血流动力学指标变化调节静脉麻醉药的用量。记录麻醉时间、血流动力学指标、异丙酚与瑞芬太尼用量。结果2组患者的一般情况、手术时间差异无统计学意义(P>0.05)。右美托米啶组患者较对照组手术中异丙酚[(3.72±0.21)mg/(kg·h)us(6.04±0.41)me,/(kg·h)]与瑞芬太尼[(0.11±0.01)(kg·min)us(0.18±0.02)(kg·min)]的用量显著减少(P<0.05)
3、。3h以上的手术,对照组费用明显多于右美托米啶组[(1740.004-544.06)元(1115.43±164.74)元,P<0.05]。结论术中持续静脉输注右美托咪啶0.6(kg·h)能显著减少手术中静脉麻醉的用药量,对于麻醉时间超过3h以上的手术可明显减少药物费用。【关键词】右美托咪啶;静脉麻醉;持续输注;丙泊酚;瑞芬太尼;用药量[中图分类号】R971.2[文献标志码】A[文章编号】1008—8199(2014)03-0268-04Thedosageeffectofcontinuousinfusionofdexmedetom
