1、脑缺血后短T1高信号的临床意义作者:张绍伟麦卫国罗汉超【摘要】提高对脑缺血后出现短T1高信号的认识并探讨其临床意义。方法:分析23例患者脑梗塞后出现短T1高信号的MRI表现,全部病例均同时行CT检查和随访。结果:病变位于基底节区15例,皮、髓质区5例,小脑3例;基底节区病变呈斑片状短T1信号,皮、髓质区及小脑病变呈散在斑点状短T1信号改变;23例MRI检查出现短T1信号后,立即行CT检查,22例病变中无高密度影,1例枕叶梗塞部位出现少许点状高密度影与MR图像相似。结论:脑缺血后出现短T1高信号并非出血,而是多种因素综合所致,这对临床正确治疗选

6、tors,soitisveryimportantfortheclinicaltreatment.[KEYWORDS]BrainIschemia,BrainInfarction,MagneticResonanceImaging,ShortT1Signal众所周知,脑梗死在T1WI上表现为低信号,T2WI上表现为高信号影。但在临床工作中,我们遇到少部分病人在脑血管意外发作后3~12h急查CT没有发现出血灶,而第二天复查MRI发现梗塞灶内出现斑点状短T1高信号,立即再复查CT,梗塞灶内未见高密度出血。同样在一部分脑外伤病人中,出血吸收后,CT检查为
7、阴性,而MRI检查水肿软化灶中见短T1信号影,故对短T1信号的正确认识关系到临床的正确诊断和治疗。1资料与方法1.1一般资料2005.1-2007.10月在我院行MRI检查的脑梗塞病人,脑缺血后出现短T1高信号的23例病例,其中男14例,女9例;年龄41-86岁,平均年龄66岁;临床表现为头痛、头晕,口角歪斜,肢体乏力,肌张力降低等。1.2检查方法使用GESigna0・2T永磁型MRI成像仪,头颅线圈扫描,患者仰卧位,头先进,磁场中心定于听眉线,扫描序列包括:(1)自旋回波(SE)T1WI序轴、矢或冠位成像(TR/TE340/21),层厚8m
8、m,层距2mm,矩阵:256/192,NEX:4,FOV24mmx18mm,(2)快速自旋回波(FSE)T2WI序列横断位成像(TR/TE3500/94.5),层厚
