1、硬度指数及脉搏波速度评估正常血压高值者外周动脉弹性阚艳敏马琳1刘洋刘水澎李建王艺桦华北理工大学附属医院超声科河北唐山063000【摘要】目的应用血管回声跟踪技术观察正常血压高值者颈动脉及股动脉弹性,探讨硬度指数(卩)及单点脉搏波速度(PWVP)评价外周动脉弹性的价值。方法二维超声测量正常血压高值组(76例)与正常对照组(50例)双侧颈动脉及股动脉内屮膜厚度(IMT),并应用ET技术检测动脉卩及PWVp,对比分析两组参数;运用ROC分析法进行统计学分析。结果正常血压高值组的卩、PWVP较正常对照组增
2、高(P<0.001),而两组间动脉内中膜厚度差异无统计学意义(P>0.05);ROC分析显示颈动脉卩、PWVP和股动脉卩、PWVP的曲线下面积分别为0.762、0.861、0.778、0.891,以颈动脉卩7.45、PWV卩6.05m/s用口股动脉卩7.55、PWVp6.15m/s作为正常高值者外周动脉弹性异常的诊断界值时,敏感度与特异度均较高。结论正常血压高值者的外周动脉弹性在内中膜未出现形态学改变前已经降低,颈动脉卩7.45、PWV卩6.05m/s和股动脉卩7.55、PWV卩6.15m/s能够
