1、MaterialTransferAgreementfortheTransferofOrganisms(MTA-TO)toAcademic/Not-for-ProfitOrganizationsInresponsetoRECIPIENT’srequestfortheMATERIAL(specifically,thenameofthegeneorallelemutationthatmakestheorganism(s)unique)________________________________________________________________________________

2、________________________________________________________________________________________________,foundwithinthe__________________[organismstrain,species],andanyunmodifiedderivativeandunmodifiedprogeny,aswellasanybiologicalmaterials(including,withoutlimitation:zygotes,embryos,cells,tissues,fluids

3、,etc.)whichcontainorincorporatetheMATERIALandarederiveddirectlyfromtheoriginalorganismoritsunmodifiedprogeny,tobeusedforthepurposeof:___________________________________________________________________________________________________________________________________________________________________

4、______________________________________________________________________________________________________________________________________ThePROVIDERrequiresthattheRECIPIENTagreetoandtheRECIPIENTSCIENTISTacknowledgethefollowingtermsbeforetheRECIPIENTreceivestheMATERIAL:1.TheaboveMATERIAListhepropert

5、yofthePROVIDERandismadeavailableasaservicetotheresearchcommunity.2.THISMATERIALISNOTFORUSEINHUMANSUBJECTS.3.TheMATERIALwillbeusedforteachingornot-for-profitresearchpurposesonly.4.TheMATERIALwillnotbefurtherdistributedtootherswhoarenotundertheRECIPIENTSCIENTIST’sdirectsupervisionwithoutthePROVIDE

6、R’swrittenconsent.TheRECIPIENTshallreferanyrequestfortheMATERIALtothePROVIDER.Totheextentsuppliesareavailable,thePROVIDERorthePROVIDERSCIENTISTagreetomaketheMATERIALavailable,underaseparateMaterialTransferAgreementfortheTransferofOrganismstootherscientistsforteachingornot-for-profitresearchpurpo

7、sesonly.5.TheRECIPIENTagreestoacknowledgethesourceoftheMATERIALinanypublicationsreportinguseofit.6.AnyMATERIALdeliveredpursuanttothisAgreementisunderstoodtobeexperimentalinnatureandmayhavehazardousproperties.THEPROVIDERMAKES
