3、及学位答辩情况表2贵阳医学院2013届硕士研究生论文缺血性脑卒中认知功能障碍的临床研究专业:神经病学,研究生:王复生,导师:朱祥路副教授摘要目的:了解缺血性脑卒中后认知功能障碍(PSCI)的发生率及相关危险因素,探讨神经功能损害与梗死部位的关系,对卒中后痴呆做出早期预测,指导康复治疗。方法:选取贵阳医学院附属医院2012.4~2013.3住院的急性缺血性脑卒中患者175例为研究对象,对其危险因素、卒中部位、神经损伤状况、日常生活能力及认知状况进行调查,以蒙特利尔认知评估量表(MoCA)得分结果是
4、否小于26分为标准进行分组,经统计分析,了解急性缺血性脑卒中发病后3个月认知功能障碍发生率的状况、相关危险因素、卒中部位与认知功能障碍的关系。结果:1.认知功能障碍的发生率在发病1周内为57.71%,第2周时为52.57%,第12周时为36.57%;2.认知功能正常组(N-VCI)平均年龄小于血管性认知功能障碍(VCI)组,受教育年限高于认知功能障碍组(P<0.05);3.单因素分析显示:两组间高血压、糖尿病、高同型半胱氨酸、高胆固醇血症差异具统计学意义(P<0.05),吸烟、饮酒、高甘油三酯血
5、症、房颤差异无统计学意义(P>0.05);4.多因素回归分析显示:两组间高血压、糖尿病、高同型半胱氨酸血症、高胆固醇血症、受教育程度、年龄差异均有统计学意义(P<0.05);5.额叶、颞叶及脑白质病变在两组之间的差异有统计学意义(P<0.05);6.卒中后3个月时两组间NIHSS、ADL评分的差异有统计学意义(P<0.05)。结论:发病后3个月内,认知功能障碍的发生率呈下降趋势;年龄与患者认知呈负相关性,教育年限对于认知功能障碍是保护因素;高血压、糖尿病、高同型半胱氨酸血症、高胆固醇血症、受教育
