3、名称医学影像与核医学研究生张千里导师郭晓山中文摘要目的:使用双入口CT灌注成像的方法,分析研究原发性肺癌的血供情况及血流信息。材料与方法:共收集肺癌共34例(男22例,女12例,年龄30-72岁,平均年龄54岁),其中鳞癌17例,腺癌13例,小细胞癌4例;中央型15例,周围型肺癌19例。上述病例中,23例首诊时已在外院确诊为肺癌,确诊方法分为纤维支气管镜检查并活检、穿刺活检;其中14例在我院手术得到进一步证实。另外11例入院后确诊,其中5例为支气管镜检查并活检确诊,3例为穿刺活检确诊,3例术后病理确诊。对此34病例肺部肿块行CT灌注扫描。应用机器为东芝公司Aquil
4、ionPRIMECT扫描仪(双能CT,80排螺旋CT),对肺部肿块连续动态灌注容积扫描,连续扫描27.5秒。造影剂选用碘佛醇注射液,造影剂注射采用4ml/s;注射剂量为60ml。图像采用东芝公司提供的bodyperfusion5软件包后处理,采集肿块的支气管动脉灌注血流量(arteriaebronchialebloodflow;AF)、肺动脉灌注血流量(pulmonaryarterybloodflow;PF),并计算出灌注指数(PI:支气管动脉灌注血流量占总血液量的百分比,AF/(AF+PF))。同时,得到病变区域血流信息的伪彩图。结果:1、所有病例中,AF最大值为
5、194.4ml/min/100ml,最小值为37.8ml/min/100ml;平均值为96.53±38.84ml/min/100ml;PF最大值为123ml/min/100ml,最小值为10.6ml/min/100ml,平均值为44.91±32.06ml/min/100ml;灌注总量最大值为317.4ml/min/100ml,最小值为69.5ml/min/100ml,平均值为134.4±59.2ml/min/100ml。2、灌注指数(PI)最大值为87.29%,最小值为55.62%,平均值为70.60%±7.83%。肺鳞癌PI平均值为71.02%±4.51%,腺癌P
6、I为69.%75±8.16%,肺小细胞癌72.59%±9.94%。三者间两两对比,P>0.05,差异无统计学意义。中央型肺癌PI平均值为71.08%±4.18%,周围型肺癌PI为70.23%±9.14%,两者之间对比,P>0.05,差-1-贵阳医学院2010级硕士研究生论文异无统计学意义。结论:双入口CT灌注成像能定量评价肺部病灶体循环和肺循环的血流量,是无创性研究肺部病灶血流动力学的有效工具;对原发性肺癌血供模式的初步研究提示,肺癌同时接受肺循环和体循环供血,其中,支气管动脉血供占总血供的70.60%±7.83%。关键词:CTCT灌注成像肺癌灌注指数-2-贵阳医学
