3、tofyourneedsforcommunitycareservicesandhasagreedthatthesupportidentifiedintheattachedSupportPlanshouldbeprovided.b.TheCountyCounciliswillingtomakeaDirectPaymenttoyouoryourrepresentativetoenableyoutosecurethesupportidentifiedinyourSupportPlanc.Youoryourrepresentativeacknowledgethatyourneedfo

4、rcommunitycareservicesmaychangefromtimetotime,inwhichcasethesupportdescribedinyourSupportPlanmaychangeaccordinglyd.YouoryourrepresentativearewillingandabletosecurethesupportidentifiedinyourSupportPlane.TheCountyCouncilhasprepareda‘SupportingDocument’entitled‘DirectPayments–Managingyourownca

5、reandsupport’describinghowtheDirectPaymentsSchemewillwork.TheSupportingDocumentformspartoftheAgreementandyouoryourrepresentativeagreetocomplywithit.8DP1SDST14.10.081.TheDirectPaymenta.TheCountyCouncilwillpayyouthesumof£perweek(the“DirectPayment”).Thesepaymentswillbemadeasperthepaymentschedu

6、lesupplied.TheDirectPaymentmaychangefromtimetotimeinaccordancewithclause7.b.TheDirectPaymenthasbeencalculatedtoincludeanassessedchargefromtheServiceUserof£perweek,whichrepresentsyourcontributiontothecostoftheServices.Thischargemaychangefromtimetotimeinaccordancewithclause7.c.AnyLumpSumDirec
