1、胸腰椎爆裂骨折的治疗综述李定文郑均华湖北中医药大学湖北省第三人民医院摘要：胸腰椎爆裂骨折是由垂直压缩暴力所致的损伤，占胸腰椎骨折的50%以上，伤后椎体粉碎、爆裂，如椎体后壁骨碎片移位突入椎管易致脊髓或马尾神经损伤。随着现代化工业、交通运输业、建筑业及体育事业的快速发展，意外事故的发生率也随之上升，因此胸腰椎骨折目前呈现高发生率、高致残率、高治疗费等特点，但对其治疗方法的选择目前尚有争议。因此，选取一个有效而适合的治疗方法对患者的治疗括减少并发症的发牛.，有着重要而深刻的意义。对于不同类型的骨折其治疗方法各不相同，0前应

6、,theacceptedmethodsincludeconservativetherapy,posteriorreductionandinstrumentation,anteriordecompressionandinstrumentation,combinedanterior-posteriorapproachandminimallyinvasivesurgery.However,theoptimaltechniqueisstillnotclear.Surgicalapproachgenerallydependson

8、划分为两大类型，即稳定性胸腰椎损伤和不稳定性胸腰椎损伤，其认为稳定性损伤不必进行复位固定治疗，而不稳定性胸腰椎损伤则必须行复位固定治疗。上世纪70年代Holdsworth[2]在Nicol1的分类方式的基础上，将胸腰椎骨折进一步细分为四型，其认为是否为稳定性损伤应由伤后后方韧带复合结构是否完整来决定，并第一次提到了“胸腰椎爆裂性骨
