1、小剂量罗哌卡因复合芬太尼与等剂量单纯罗哌卡因腰-硬联合麻醉用于急诊剖宫产的临床观察呂燕(建水县妇幼保健院云南建水654399)【摘要】A的:观察小剂量罗哌卡因复合芬太尼蛛网膜1腔阻滞,与应用等剂量单纯罗哌卡因相比,能否更为安全、有效地用于急诊剖宫产术。方法:2014年5月一2014年9月在本院进行急诊剖宫产的单胎产妇60例,随机分为罗哌卡因复合芬太尼组和单纯罗哌卡因组,产妇左侧卧位下于L3-4间隙用针
2、Aj针方法行腰-硬联合麻醉,分别用罗哌卡因复合芬太尼或单纯罗哌卡因蛛网膜卜'腔注入。记录患者麻醉前、给药后心率、血压、呼
3、吸变化,麻醉平面、麻醉起效时间、T肢运动神经阻滞情况,硬膜外追加局麻药、使用升压药、术屮疼痛、肌松不满意情况,记录新生儿体重、出虫1minAPgar评分,产P出血量。结果:麻醉平面复合芬太尼组高于单纯罗哌卡因组(P<0.01),麻醉起效时间复合芬太尼组长于单纯罗哌卡因组(P<0.05),血压麻醉后较术前降低(P<0.01),给药后5min、7min单纯罗哌卡因组低于复合芬太尼组(P<0.01,P<0.05),呼吸频率复合芬太尼组给药后较术前降低(P<0.01,P<0.05),使用
4、升压药单纯罗哌卡因组高于复合芬太尼组(P<0.01),关腹痛单纯罗哌卡因组高于复合芬太尼组(P<0.05),皮肤瘙痒复合芬太尼组高于单纯罗哌卡因组(P<0.01),呼吸频率、运动评分组间比较差异无统计学意义(P>0.05)o结论:罗哌卡因复合芬太尼蛛网膜下腔阻滞与等剂量单纯罗哌卡因蛛网膜下腔阻滞相比,麻醉效果更加确切,产妇血流动力学更加平稳、关腹时牵拉痛的发生率明显降低,镇痛更完善。【关键词】罗哌卡因;芬太尼;腰-硬联合麻醉;急诊剖宫产【屮图分类号】R614【文献标识码】A【文章编号】2095-1
