1、微造瘘经皮肾穿刺钬激光加吸附系统治疗上尿路结石作者：马继乔，张伟伟，刘兵，周业茂，辛昂【摘要】目的探讨微造瘘经皮肾穿刺输尿管镜钬激光加吸附系统碎石取石术（mini-PCNL）的疗效。方法对102例患者接受mini-PCNL治疗上尿路结石的临床资料进行回顾性分析。结果102例患者中100例成功施行一期碎石取石术，2例中转开放手术，一期取净结石95例，5例有小残石，随访4周后4例结石排净，平均手术时间1h，肾造瘘管留置平均时间7天，平均住院8天，未出现严重并发症。结论mini-PCNL具有损伤小、恢复快，钬激光加吸附系统具有碎石快、颗粒小、清除率高、并发症少等优点，

6、）具有切口小、出血少、结石清除率高、恢复快等优点，被认为是治疗肾及输尿管上段结石的理想方法。近年来医用钬激光治疗机及吸附系统的临床应用，明显缩短了手术时间，减少了结石的残余率及并发症的发生。我院自2008年8月—82009年8月共收治102例肾、输尿管上段结石患者，采用微创经皮肾穿刺输尿管镜下钬激光加吸附系统碎石取石术，取得了良好的效果。现报道如下。　　1资料和方法　　1.1临床资料　　本组102例，男68例，女34例，年龄21～75岁，平均45岁。单纯肾结石70例，肾结石并发同侧输尿管上段结石5例，肾结石并发输尿管中下段结石7例，单纯输尿管上段结石20例（本组

7、资料不包括对侧肾及输尿管存在病变的6例）。结石直径1.5～6.5cm，平均3.0cm。多发结石42例，鹿角形结石10例，阳性结石99例，阴性结石3例，81例并发中重度积水，6例患侧有上尿路结石开放手术史，36例患者曾行体外冲击波碎石术（ESWL）治疗，结石未排出，3例在外院行输尿管上段结石气压弹道碎石术，结石上移进入肾脏，手术失败。患者术前要求练习俯卧位，以减轻术中疲劳感及呼吸压迫感。　　1.2手术方法8　　采用连续硬膜外麻醉，先取膀胱截石位，患侧经尿道输尿管镜检，对合并中下段输尿管结石患者应用钬激光碎石，将结石取出。留置F5输尿管导管，保留导尿，并将输尿管导管
