OperationalSafetySupportingTool〈操作安全支持工具〉VesselInspectionsaretobecarriedoutpriortocommencingoperationsuponthevessel.VesselinspectionsmustbecarriedoutwitharepresentativefromthevesselandsignedoffbytheChiefOfficer(C/O).Additionalsafetyinspectionscanbecarriedoutduringtheshiftoraspointsofwork(POW)change,thesenewinspectionsmustbesignedoffbytheChiefOfficer(C/O)ortheDeckOfficer(D/O).巡查必须在船上工作廿展前进行及必须有一位船代表陪同下进行,检查农必须由大副签署。如在同一班有工作点〈POW〉改变了,额外巡查必须进行,木巡查表必须由大副印送往船主或其代表。Allunsafeconditionsraisedduringtheinspectionmustbesupportedbyphotographic/videoevideneeandsenttothevessel/shippinglinerepresentativeelectronicallyandloggedinthevesseldatabase.如在巡查期发现一些不安全处,必须拍照或录像作证明,然后将这些数据利用电邮送往船方/船公司及记录在船方数据库内。ConditionReportReference〈巡查表序次〉Guideline〈指引〉1.General〈一般〉1.1DidthevesselreceiveavesselarrivalpackoutliningSafety,EnvironmentandSecurityrequirements《在船泊桥时,码头一方须向船方呈交「船方抵达文件夹」其内容包括安全,环境及保安之要求?》VesselArrivalpacksshallbesenttoallcallingvesselscontaining〈必须向到访船方呈交「船方抵达文件夹」,文件夹内容包扌舌下列安全,环境及保安之要求:〉-LetterofInduction职前培训信-Terminalrulesandprocedures码头之守则与规程-LocalRegulations当地法例・ContactinformationofkeypersonnelincludingPersoninCharge码头之主耍联络人之姓名及电话〈包括指令人员〉1.2AnyoutstandingNoticeofDeficienciesonvesseldatabase(attachifnecessary)〈在船只数据库有没有未完成之「不足处通知书」?〉VesselDatabaseshallbekeptandregularlyupdatedtomonitorandcontrolanyoutstandingSafetyandEnvironmentainonconformities/actionsraisedagainstthevessel.VesselDatabaseshallbeupdatedfollowingeachvesselinspectioncarriedout.〈船只数据库需保存妥当及定期更新,以便监察及控制任何船方未完成Z安全及环境违章爭宜。待每次巡查后,该船Z数据序亦必须更新。13Hasthelashingcrewundergoneasafetyinduction〈装隹卩工人是否已接受了职前安全培训?〉AllemployeesandcontractorsrepresentingDPWorldmustundergoaSafetyinductionbeforecommencingworkatanyterminal.〈任何新聘任员工或承包商工人在开展工作前必须接受职前安全培训。〉1.4Hasthelashingcrewundergonevessel/lashingsafetytraining《装卸工人是否已接受了船上打拆加固Z安全培训?》Alllashingcrewmustundergovessel/lashingsafetytrainingagainsttheirspecificjobdescription.〈装隹卩工人必须接受船上打拆加固z安全培训。〉1.5AnyfeedbackfromLashinggangorvesselcrew〈装卸工人或船员冇没有冋馈或意见?〉Comments/suggest!onsraisedbyemployees/contractorsorvesselcrewmustberecordedandpresentedtotheOperationsandSafetymanager.〈任何由员工/承包商工人/船员所提出之;评语/意见必须记录下来及呈交了操作部及安全经理解决。〉 2.Access〈进出位〉1GangwayisproperlyIandedandclearoferanesgantrypath,setcorrectlyandnetted《舷梯是否正确地摆放?是否不在铁塔〈桥吊〉Z行机〈行大机〉Z路线内?是否设置安全网在其下?》ILO7.2.1(5)Asafetynetshouldberiggedwhereeverapersonmayfallbetweentheshipandthequayfromameansofaccesstoaship・扌安国际劳工组织<ILO)第7.2.1(5)所示,安全网必须设置于舷梯与码头之桥边Z间,防止人体下坠。ILO7.2.3Gangways按国际劳工组织<ILO>第723所示1.Gangwaysshouldbeplacedatrightanglestotheship'ssideandconnectthequaywithoneoftheship'sdecksorwiththebottomplatformoftheaccommodationladder・〈舷梯必须与船身呈九十度角,舷梯Z放底部亏码头Z桥边间必须以平平台接联〉2.AGangwayshould〈舷梯必须〉:-Havecloselyboardedwalkwayatleast550mminwidth;以550mm之木板铺密;-Befittedwithtransversetreadsatsuitablesteppingintervals;在梯级间配备了横向面;・Befittedwithupperandintermediateguardrails;配豐顶及中护栏;-Befittedwithdevicesenablingittobeproperlysecuredtotheship;安全t也i帘稳;-Tobefittedwithproperslingingattachmentssoplacedthatitwillbalaneeabouttheattachmentwhenitissuspended,ifaliftingappliancehastobeusedtoplaceditinposition;设置合适吊具以稳固及平衡舷梯;-Notnormallybeusedatanangleofmorethan30°tothehorizontai,or45°ifitisfittedwithtransversetreadsevery500mmorso;不可超过水平30或每隔500mm以45。朝向横向面:-Neverbeusedatanangleofmorethan45°tothehorizontal;不可超过水平45°;・Landonaclearandunobstructedareaofthequay.摆放在没有被阻挡的桥边。3.Whereagangwayrestsonaship'sbulwark,safemeansofaccessshouldbeprovidedbetweentheship'sdeckandthegangway.放置在舷墙时,安全进出道应处于甲板利舷梯之间。4.Agangwaythatrestonaquayonrollersorwheelsshouldbeinsuchawaythattherollersorwheelsareonareasonablylevelsurfaceandnotthevicinityofanyobstructionorholethatcouldrestricttheirfreemovement・Therollersorwheelsshouldbefittedorguardedinsuchawayastopreventaperson'sfootfrombeingcaughtbetweenthemandthequaysurface,leavingaminimumgapof50mm・舷梯Z底部须设置轮子或滚轮,以便轮子或滚轮停放在一个合理的水平面上,轮子或滚轮并没有遇上任何障碍物或孔洞,让其在没有限制下自由移动°该轮子或滚轮应安装防护设施,以防止它们与码头桥边乙间的空隙可夹伤人脚〈空隙如少于50mm可以不须理会〉。5.Agangwayshouldbesecurelyfastenedtotheship.Onearrangementconsistsofplacingthegangwayabovethedeckortheplatformandprovidingafasteningatthebottom.稳固地放置在船上。可放置在甲板上或安放在平台上然后稳周其底部。6.Gangwaysshallbelandedsecurelyonquaysurfaceandnotobstructingcranetravelpathortrafficflows稳固地放置在桥面及不在桥吊之行大机之路线内。2.2Hatchcoversaboveladderscanbesecured〈在梯具.上之舱盖是否稳固?〉ILO7.4.1按国际劳工组织(ILO)第7.4.1所示(7)Lockingdevicesshouldbestrongenoughtowith-standareasonableblowfromanyswingingcargowithoutdamage・Thedevicesmaybeself-locking,sothatthelockedcomponentcanbereleasedonlybymanualoperation•锁定装置应具有足够的强度,以能够承受来自任何摆动的货物的合理碰撞而不损坏。该装置可以是自 -Haveaclearanceof750mmfromtherungtothebackofthehoop;与梯级至少有750mm相距-Beconnectedbyverticalstripssecuredtotheinsideofthehoops,thestripsbeingequallyspacedaroundthecircumferenceofthehoop.被間定到所述背环的内垂直线条连接,垂直线条被等间距围背环的圆周。2.8Provisionofsafeaccess/egresstounderdeckstows〈设置安全进出位往返船舱〉ILO7.8.2按国际劳工组织<ILO>第7.8.2所示(1)Shore-sidemanagementshouldensurethatsafeaccessisprovidedbytheshiptoanyplaceontheshipwherestevedoreshavetoworkandthattheplaceofworkissafe.桥边管理人员必须确保船方在船上提供安全进出通道予装卸工人及安全之工作点。 3.EdgeProtection〈船缘边之保护〉3.1Outboardcellsareprovidedwithsafetyrailssufficienttopreventapersonfallingoverboard《在船最外围的配载位置设置足以防止人体F坠海面Z护栏》ILO7.4.4按国际劳工组织<ILO)第7.4.4所示1.Hatchesatdecklevelshouldbeprotectedbycoamingsofsufficientheighttopreventaccidentalfallsintothehold・Coamingsshouldpreferablybelmhigh.舱口Z开启舱盖必须以足够髙度Z围板围显,以防止人体卜-坠入船舱,围板绘少有1米咼O2.Portworkersshouldnotworkoncargosondeckorbetweenthatareoveranopenhatch.码头工人不准在货箱顶或开启舱盖口旁工作。3.Workinthe'tweendeckareshouldnotnormallytakeplaceifthehatchisopentothelowerhold・Ifthiscannotbeavoided,thehatchshouldbefencedtopreventportworkersfromfalling.如舱盖开启后,在正常情况下不准在"舱内第一层"内工作。如果无法避免,舱口应设置围栏,以防止码头工人坠落舱底。4.Thefencingshouldbelmhighandmaybeofsuitablewireropeorchainprovidedthat;护栏必须为一米高或以合适之网丝绳/铁链代替・Therearemeanstokeeptheropesorchainsastautaspossible;须利用设备将网丝绳/铁链拉紧・Wireropeshavesufficientwiresperstrandtobeflexible,arefreefrombrokenwiresandanylooseendsarefittedwithferrulesorothermeansofprotectiontopreventinjury;钢丝绳每有足够的钢绞线导线及没仃断丝,任何松动的两端都配有套圈或其他保障方法,以防止令作业人员受伤;-Sufficientstanchionsareprovided.捉供足够的支柱。5.Decksocketsintowhichstanchionsfitshouldbeequippedwithlockingdevicesandshouldbesufficientlydeepanddesignedinsuchawayastopreventthestanchionsfrommovingundulyoutoftheverticalorbeingaccidentlydisplaced•支柱在卬板Z插座须设置上锁装置,插座有足够之深度,并设计了防止支柱的意外滑出之装置。6.Thefencingshouldformapermanentpartoftheship'sequipmentandbekeptinplaceatalltimes,except;护栏是构成船舶设备的永久组成部分,被保存在的地方在任何时候,除了;-Whenthehatchisbeingopenedorclosed;当舱盖被开启或关闭;・Whengoodsarebeingloadedontothatparticulardeckandtheworkinthatholdpreventsthehatchfrombeingclosed;当货物被装上特定的甲板和在作业时防止舱盖被关闭;-Duringmealbreaksorsimilarshortinterruptionsofwork.在用膳时间或类似的短暂中断。3.2Provisionforrailings(topandmid-rail)aroundopenhatchesandanyheightwherefallisgreaterthan2meters《在刃•启之舱盖及任何高于两米之地方设置安全护栏(顶及中护栏)》Seeabove.参考上述ILO7.5.2按国际劳工组织〈ILO〉第7.5.2所示10.Suitableprotect!onshouldbeprovidedwhereport-workerhavetoworkclosetoedgesfromwhichtheycanfallmorethan2m.Thismaytakeformofnettingorothersuitablemeans.当码头工人在接近边缘工作及有机会下坠超
