1、动态无创PAP测定在重症肺炎合并呼吸衰竭患儿中的应用价值分析 【摘要】目的:探析动态无创肺动脉压力(PAP)测定在重症肺炎合并呼吸衰竭患儿中的应用价值。方法:选取2012年2月-2014年10月本院收治的重症肺炎合并呼吸衰竭患儿55例作为研究对象,根据是否肺动脉高压(PAH),将其分为PAP正常组19例和PAH组36例,均实施综合治疗,对比两组患儿的机械通气使用情况以及治疗转归情况。另对PAH组加用米力农注射液治疗,对比其治疗前后PAP以及心功能指标的变化。结果:PAH组机械通气率83.33%明显高于PAP正常组
2、的36.84%,机械通气时间明显长于PAP正常组,差异均有统计学意义(P0.05)。PAH组的好转出院率为80.56%、死亡率为19.44%,PAP正常组19例患儿均好转出院,两组比较差异有统计学意义(字2=4.233,P=0.040)。结论:动态无创PAP测定用于重症肺炎合并呼吸衰竭患儿能为预后判断和病情评估提供依据,且米力农用药能有效降低PAP,值得临床应用。 【关键词】超声心电图;肺动脉压力;重症肺炎;呼吸衰竭;儿童 【Abstract】Objective:Toinvestigatetheapplicat

4、Andaccordingtowhetherthepulmonaryarterialhypertension(PAH),allchildrenweredividedintothenormalPAPgroupfor19casesandthePAHgroupfor36cases,theyweregivencomprehensivetreatment.Themechanicalventilationuseandtreatmentoutcomebetweenthetwogroupswerecompared.ThePAHgro
5、upwastreatedwithMilrinoneInjectionfortreatmentonthebasisofcomprehensivetreatment,thePAPandthechangeofcardiacfunctionindexesbeforeandaftertreatmentwerecompared.Result:ThemechanicalventilationrateofthePAHgroupwas(83.33%)significantlyhigherthan36.84%ofthenormalPA
