4、thingin(2)therightwayat(3)therighttime,(4)ALLthetime–evenwhenifnooneislooking.Thesafetyculturethatexistsinaplantorcompanyistheresultofalltheactions-andinactions-ininstitutional/workforcememory.1Manyfacilitiesusemanagementsystemstohelpcontrolrisksofhazardousprocesses.Thesemanagementsystemsareoperated

5、bypeople–peoplewhoseinherentattitudesaboutsafetycanaffectthechoicestheymakeinoperatingthesesystemsand,thus,theoverallsafetyperformanceofthefacility.Thispaperpresentslessonslearnedfromcompaniesimplementingprocesssafety/HSEcultureimprovementsinvolvingthefollowingsteps:education,planning,workforceinvol

6、vement,andprovidingremediesforcultureweaknesses.2.UnderstandingProcessSafety/HSECultureTheCenterforChemicalProcessSafety(CCPS)hascreatedaframeworkforunderstanding,evaluating,andimprovingculture(Table1)2,3.Usingthatframework,wehavecreatedauniqueanalysisprocessfordeterminingcultureweaknessesbycomparin

7、gworkforceopinionsgainedviasurveyandinterviewswithHSEperformancedeficiencies.4ThisPerformanceAssuranceReview(PAR)approachwaspresentedattheMay20081stLatinAmericanProcessSafetyConferenceinBuenosAires.Th
