2、ubmittedtoyourmanagerapproximately2weekspriortoyourPerformanceAppraisaldiscussion.ThePerformanceReviewFormprovidesperiodicwrittenreviewofindividualperformance,inthecontextoftheongoingperformancemanagementprocess.Itisdesignedtofacilitateconstructivediscussionb
3、etweentheemployeeandmanagerinordertoclarifyperformanceobjectives,providefeedbackabouttheemployee’sperformancewithrespecttoskillsandbehaviors,provideaframeworkforidentifyingtheemployee’sdevelopmentplans,andserveasabasisformeritincreasedecisions.Managersandempl
4、oyeesareresponsibleforcompletingayearlyperformancereviewaspartofongoingperformancediscussions.Preparationfortheperformancereviewdiscussionshouldbeginwiththeemployeecompletingaself-appraisal.TheHumanResourcesDepartmentisavailabletoanswerquestionsandtoprovideas
5、sistancetomanagersandstaffmembersonanyaspectoftheperformancemanagementprocess,includingtheperformancereviewform.IMajorAreasofResponsibility(Thisiswhatyoudo)ThissectionofthePerformanceReviewformisusedtorecordthethreeorfourmajoractivitiesorgoalsthatyouarerespon
