1、提要目的:通过对山东中医药大学附属医院213例恶性肿瘤住院患者情况的分析,探讨恶性肿瘤患者肿瘤相关性抑郁、焦虑的发病情况及影响因素,为临床治疗提供借鉴。方法:对住院患者进行问卷式临床调查,采用SPSS17.0软件进行数据统计,分析恶性肿瘤住院患者肿瘤相关性抑郁、焦虑的发病情况及各因素对抑郁、焦虑发病率的影响,找出中医证型和肿瘤相关性抑郁、焦虑发病的相关性及影响患者肿瘤相关性抑郁、焦虑发病的独立危险因素。结果:肿瘤相关性抑郁发病率为46%,其中中度抑郁发病率为18.3%,未发现重度抑郁者;肿瘤相关性焦虑发病率为16.4%,其中中度焦虑发病率为6.
2、6%,未发现重度焦虑者。肿瘤相关性抑郁与肿瘤发病部位、肿瘤家族史、居住地无关(P>0.05),与性别、年龄、肿瘤分期、中医辨证分型、慢性癌痛、KPS评分、化疗方案种类、治疗副作用、知情情况、费用负担方式、文化程度、收入相关(P<0.05),其中性别、中医辨证分型、肿瘤分期、既往化疗史、知情情况、慢性持续性疼痛、KPS评分等7个为独立危险因素。中医辨证分型中气郁化火、肝气郁结、痰气郁结发病率依次为95%、92.1%、61.2%。结论:肿瘤相关性抑郁、焦虑临床发病率较高,对女性、中晚期、有多次化疗史、KPS评分低、伴慢性癌痛者,应重视其心理状况,及
