1、急性阑尾炎彩色多普勒超声影像图像特征及诊断价值滨海县五汛镇中心卫生院江苏盐城224532摘要:目的:探讨急性阑尾炎的彩色多普勒超声声像图特征及诊断价值,为不断提高急性阑尾炎的超声诊断准确率。方法:随机抽取我院2013年2月-2015年12月间经临床手术证实26例急性阑尾炎患者超声存图进行回顾性分析。结果:急性单纯性阑尾炎17例,占65.38%,表现为腊肠样无回声带可伴有粪石嵌顿,横断面呈现“H全食征”,阑尾根部可见点状及短条状血流信号;急性化脓性阑尾炎3例,占11.54%,表现为阑尾明显肿大,管壁毛糙、厚薄不均,横断面可见“靶环征”,其周围可见不规则液性暗区;坏疽性及穿孔性阑
2、尾炎2例,占7.69%,表现为阑尾正常管状结构消失,周围见模糊的低回声暗区。当阑尾穿孔时,阑尾壁连续性中断,阑尾腔内可见气体强回声及阑尾腔内的无回声与周围的无回声相沟通;阑尾周围脓肿4例,占15.38%。腔内可有积气,右下腹积液,周围肠系膜水肿增厚和大网膜包裹包块,穿孔并发腹膜炎时可引起肠管扩张、蠕动弱或消失。结论:根据彩色多普勒超声成像图能较为准确的判断急性阑尾炎及其各种类型,为临床提供有利的诊疗依据。关键词:急性阑尾炎;彩色超声多普勒超声;诊断Acuteappendicitiscolorimageimagecharacteristicsanddiagnosticvalue
