7、38中文摘要公式测热法与间接测热法测定重症机械通气病人能量消耗的比较摘要目的:营养支持治疗已经成为和循环支持、呼吸支持并称的重症患者的三大支持手段。营养不良及营养过剩等在重症患者营养支持治疗中是常见的并发症,长时间营养支持的过量会导致多种并发症,如高脂血症、高糖血症、高胆固醇血症等,喂养不足会导致低蛋白血症等营养不良;即使短期内营养支持的过量或不足同样会对病人造成不良影响,因此,准确评估病人的营养需求是必要的。间接测热法(IndirectCalorimetry,IC)被认为是评估病人能量消耗(EnergyExpendi

8、ture,EE)的金标准,根据准确的评估可指导临床医师进行营养支持治疗,改善病人的预后。临床工作中通常采用的能量公式均源于国外资料,并均由非重症病人群体衍生而出,由于病人种族和生理状态的不同,根据公式计算的热量与重症病人的需求常存在很大差异,导致重症病人能量供给不足或过多。本研究通过与间接测热法对比,选取或推导与间接测热法接近的适
