3、COX-2在HP感染与非感染性胃溃疡及胃癌的表达。方法:用免疫组化方法检测196例经胃镜和组织病理学检查患有胃溃疡(病理分型:肠上皮化生、异型增生)、胃癌及大致正常胃粘膜者的胃粘膜COX-2蛋白的表达,比较各病理分类之间及HP感染与非感染之间COX-2蛋白表达的差异。结果:本研究发现肠上皮化生、异型增生、胃癌组的炎症细胞、腺上皮细胞、癌细胞及极少量正常黏膜上皮细胞中有COX-2表达。从肠上皮化生—异型增生—胃癌COX-2的阳性表达有逐渐增强的趋势(P<0.05)。胃癌组COX-2的阳性表达高于胃溃疡组(P<0.05),Hp阳
4、性胃癌组COX-2的阳性表达高于Hp阳性胃溃疡组(P<0.05),Hp阴性胃癌组COX-2的阳性表达也高于Hp阴性胃溃疡组(P<0.05)。各Hp阳性实验组COX-2的阳性表达显著高于Hp阴性(P<0.05),各Hp阳性实验组COX-2的阳性表达有差别(P<0.05),Hp阳性胃癌组COX-2的阳性表达高于Hp阳性胃溃疡组(P<0.05),而且Hp阳性胃溃疡组COX-2的阳性表达高于Hp阴性(P<0.05)。结论:①COX-2通常在正常组织中不表达,只有在受到各种物质如炎症介质、细胞因子、促癌剂等刺激下在炎症部位表达。②从肠
5、上皮化生—异型增生—胃癌COX-2的阳性表达有逐渐增强的趋势,COX-2的过度表达参与了胃癌前期病变和胃癌的发生发展过程。③Hp感染可诱导COX-2的过度表达。④COX-2在Hp感染的胃黏膜中过度表达,有加剧炎症反应和加快从浅表性胃炎—肠3化、异型增生—胃癌这一过程的作用,Hp感染可能通过诱导COX-2过度表达参与胃癌发生发展的早期过程。关键词:COX-2;胃溃疡;胃癌;Hp4COX-2ingastriculcer、gastriccancerandHPinfectionoftheexpressionandresearchof
