1、髂腹下―髂腹股沟神经阻滞联合经腹横筋膜平面阻滞与单纯经腹横筋膜平面阻滞用于剖宫产术后镇痛效果比较4分，静脉给予曲马多或肌注杜冷丁镇痛。患者术后48h内评估静息及运动NRS评分，阿片类药物使用量；记录首次下地及排气时间；记录恶心、呕吐、过度镇静的发生情况及患者对术后镇痛方式的满意度。结果所有患者腰Petit三角处解剖结构在超声下均清晰可见。C组除2例患者在髂前上棘（ASIS）附近IIIH神经均清晰可见，这2例患者在ASIS内侧1～3cm处腹内斜肌与腹横肌之间可视及旋髂深动脉，在超声下无法视及IIIH神经时将局麻药注射于旋髂深动脉附近即可。C组有1例患者出现了单侧

2、股神经阻滞症状，并伴有直腿抬高不能，运动阻滞于术后8h时消退，而股神经支配区感觉阻滞持续至术后24h。两组患者术后各时点静息及运动NRS评分差异无统计学意义（P>0.05）。两组患者术后各时间间隔（0～12h、>12～24h、>24～36h及>36～48h）比较，曲马多及杜冷丁使用量差异均无统计学意义（P>0.05）。两组患者首次下地时间、排气时间、患者对镇痛方式满意程度，恶心、呕吐发生率及镇静评分比较，差异均无统计学意义（P>0.05）。结论TAP阻滞时联合使用IIIH神经阻滞并未改善TAP阻滞于剖宫产术后的镇痛效果。　　[关键词]妇科；区域阻滞；腹横筋膜平
