4、landservicemissions;andhaveimplementedorintendtoimplementNationalCommitteeforQualityAssurancetier-2requirementsforapatient-centeredmedicalhome.Thepatient-centeredmedicalhomeisapracticemodelthateffectivelysupportsthecorefunctionsofprimarycare,useselectronicmedicalrecords,andemphasizespreventionandth

5、emanagementofchronicdisease.Qualificationcriteriafortheseprogramshavebeendescribedindetailelsewhere.Primarycareresidentswouldbetheprincipalprovidersofprimarycareservices,inclosepartnershipwithappropriatefaculty,duringa12-monthblockofclinictrainingasathird-yearresident.Ideally,first-andsecond-yearre

6、sidentswouldbeassignedtotheteachinghealthcentersfortheircontinuityclinics.Then,asthird-yearresidents,theywouldbewellgroundedincliniclogisticsandcapableofperformingasaneffectiveteamleader.Third-yearresidentswouldworkinapracticethatemphasizedcontinuityofcare,withrobustfacultysupportforthedevelopmento
