4、hcenterwiththecapabilitytoexpandandstaffthecenter,aswellasbepartofacommunitygovernanceboardcommittedtosupportingboththeeducationalandservicemissions;andhaveimplementedorintendtoimplementNationalCommitteeforQualityAssurancetier-2requirementsforapatient-cent

5、eredmedicalhome.Thepatient-centeredmedicalhomeisapracticemodelthateffectivelysupportsthecorefunctionsofprimarycare,useselectronicmedicalrecords,andemphasizespreventionandthemanagementofchronicdisease.Qualificationcriteriafortheseprogramshavebeendescribedin

6、detailelsewhere.Primarycareresidentswouldbetheprincipalprovidersofprimarycareservices,inclosepartnershipwithappropriatefaculty,duringa12-monthblockofclinictrainingasathird-yearresident.Ideally,first-andsecond-yearresidentswouldbeassignedtotheteachinghealth

7、centersfortheircontinuityclinics.Then,asthird-yearresidents,theywouldbewellgroundedincliniclogisticsandcapableofperformingasaneffectiveteamleader.Third-yearresidentswouldworkinapracticethatemphasizedcontinuityofcare,withrobustfacultysupportforthedevelopmen
