3、tteGuerinvaccinehasbeenvaccinated,theinfantshallbereviewedonthefirstMondaywhentheinfantwasbornfor4monthsGuidetoParents1.AccordingtotheregulationsofArticleXIIof“LawonthePreventionandControlofInfectiousDiseasesofPeople’sRepublicofChina”and“TheStateImplementsImmunizationCertificate
4、System”,ourcitywilluniformlyuseShenyangImmunizationRecord.2.Immunizationrecordistheeffectiveproofforchildrentoreceivevaccinationaccordingtothelaw.Whenthenewbornisborn,theparentsorguardiansshalltimelyapplytotransacttheimmunizationprocedurestothedeliveryunitorimmunizationmedialhea
5、lthunit.3.Foreachimmunizationofthechildren,theparentsorguardiansshallholdthiscertificatetovaccinateforchildrenindesignatedlocation,vaccinationoutpatientclinic,vaccinationstation,orvillagehealthcenter.4.Childrencanenrollinanursery,kindergarten,school,orgoingabroadbythiscertificat
6、e.Ifthechildrendonothavethiscertificateordonotvaccinateaccordingtothiscertificate,theymustre-registerthiscertificateandre-vaccinate,otherwisetheformalitieswillnotbetransacted5.Whenthehouseholdregistrationsofthechildrenaremigratedorliveinnewregionsawayfromhome,theparentsorguardia
7、nsshallholdthiscertificatetotransacttheimmigrationformalitiesinthedesignatedlocation,vaccinationoutpatientclinic,vaccinationstation,orvillagehealthcenter,andreturningtotheplaceoriginalresidencetotransacttheemigrationformalities6.Thiscertificateshallbekeptwell.Ifthecertificateisd
