2、associatedwithoutpatientpharmacotherapymayrequirehospitalization.However,suchhospitalizationmaybepreventedifpharmacist'spharmaceuticalcare(PC)isgiven.Weinvestigatedthereasonsforhospitalizationinmedicalinstitutionsandmedicalexpenseswerecalculated.Inpatientdiagnoses,tre
3、atment,etc.inthepreviousyearinthepastwereexamined,andcasesofhospitalizationduetodrugtherapywereextracted.Next,thepossibilityofpreventinghospitalizationwithPCpracticewasexamined.Among1552cases,outpatientpharmacotherapywasthereasonforhospitalizationin27cases.Noncomplian
4、cewastheunderlyingcauseinabout40%ofhospitalizations.Itwasthoughtthatin22caseshospitalizationcouldhavebeenpreventedbypharmacist'sPC.Theaveragehospitalizationmedicalexpensewas¥295,805perpatient.Itisnecessarytoperformregularconsultationrecommendations,interventionswithth
