1、ConfinedSpaces密闭空间1.1AuthorizedEntrants准入者Authorizationisgivenonlytoperson(s)(i.e.,employees,contractors,customers,and/orvisitors)whohavebeentrained,beendesignatedbytheSectionSupervisorandhavecompletedareviewofthePTWfortheworktobeperformedinthespace・TheCompetentPersonand/or(Beforeentryauthor

2、izationisgiven)theSectionSupervisorshallensurethathe/sheismadeawareofpersonnelthathaveanymedicalconditionsthatmustbedisclosed.Personnelwiththefollowingmedicalconditionsshallnotbeallowedtoworkinconfinedspaceswithoutpriorauthorizationbyacompanymedicaldirector.Itisrecommendedthatthisbeaccomplis

3、hedthroughtheissuaneeofaconfinedspacemedicalclearaneecertificate.仅批准接受过培训、由部门主管指定并完成了用于该空间执行作业的作业许可证审查的人员(即：员工、承包商、客户和/或访客)。有资质的人员和/或(批准进入前)部门主管须确保人员知道必须公开的医疗状况。未经公司医疗主任事先批准，冇下列医疗状况的人员不允许在密闭空间作业。建议通过签发密闭空间体检合格证來解决这一问题。•Historyoffainting,blackoutsorseizures冇昏厥、暂时失去知觉或癫痫病史•Historyofheartdiseas

4、eorheartdisorders有心脏病或心律紊乱史•Highbloodpressure高血压•Uncontrolleddiabetesmellitus无法控制的糖尿病•Breathingdifficultyduetoemphysema,asthma,bronchitis,orotherrespiratorydisorder肺气肿、哮喘、支气管炎或其它呼吸系统疾病造成的呼吸困难•Hearingdisorderthatmayaffectrecognitionofalarmsorverbaldirections可能会影响识别警报或口头指令的听力障碍•Meniere'sdiseas

5、eorotherdisordercausingalossofbalaneeorvertigo美尼尔氏综合症或造成平衡失调或眩晕的其它障碍•Claustrophobiaorothersimilarnervousormentaldisorder幽闭恐怖症或其它类似的神经功能紊乱或心理障碍•Backorjointpainorstiffnessthatmaylimitmobility可能会限制移动的背部或关节疼痛和僵硬•Deformityordisorderofthelowerlimbswhichmaylimitmobility可能会限制移动的下肢畸形或下肢异常•Seriousdefe

6、ctineyesight视力严重缺陷•Defectivesenseofsmell嗅觉缺陷•Bodymassincompatiblewithsafeingresstooregressfromtheconfinedspace体质不适合安全地进入或离开密闭空间1.2SectionSupervisors部门主管TheSectionSupervisorforconfinedspacesarethosepersonsdesignatedasSectionSupervisorsbecausetheydirectlysupervisetheAuthorizedEntrantsandensure

7、allpermitrequirementsarefollowed・TheyarerequiredtoreviewallpermitrequirementswiththeAuthorizedEntrantspriortoauthorizingentryintothespace・密闭空间的部门主管是因为他们肓接监督准入者并确保许可证的所冇要求得以遵守而被指定为部门主管。在批准进入空间前，要求部门主管与准入者一起回顾所有许可证要求。1.1GasTester/CompetentPersons气体检测
