2、案。3•临床医学硕士专业学位研究生按照个人培养计划完成相关科室的轮转，应及时、如实记录，对病例的记录须由指导教师和考核者签名。4.临床医学硕士专业学位研究生轮转出科时将本手册提交考核小组审核并签署意见。5•手册须用钢笔或碳素笔填写，字迹清楚，不得遗失，撕页和涂改。手册记录内容将作为考核的依据。6.该表格供临床各专业研究生在札I关科室（含内科、外科、妇产科、神经病学、耳鼻咽喉科、眼科、儿科、皮肤病与性病学、老年科、康复医学、精神病与精神卫生学）轮转记录使用以及临床各专业研究生在本学科训练时记录使用。7•符合免轮转条件的在职
3、临床医师，不需要填写相关轮转科室的记录表格，但需要提交完成本学科的记录手册和免轮转科室出科考核成绩。同时，在表格的最后，需耍提供以下证明材料的复印件：在三级乙等及以上医院完成住院医师规范化培训第…阶段的合格证书（或成绩证明）。Instructions1Thepostgraduatesmajoringinclinicalmedicinemustdotheswifttrainingandassementinrealateddepartmentsaccordingtotherequirementsofperaonalculti
