1、提要目的:研究提高和提前对脑梗死前期的诊断水平,旨在减少脑梗死的发生。方法:由于对比增强磁共振脑灌注成像(PwI)在脑缺血诊断中具有较为敏感的特点,用此技术及弥散加权(DwI)和常规MRI技术对18例脑缺血患者和20例非脑血管病患者进行脑梗死前期的对比分析、梗死前期分期和诊断方程的研究。结果:在六种MI己I成像技术中,仅有脑灌注成像可以检出脑梗死前期病例,余均不能检出。脑梗死前期分期特点为:Il期TTP轻度延长,12期MTT延长,III期CBF下降,112期CBV下降。诊断回归方程:Y一0.152+0.295Xl+O.332X2+O.076X3+0.187X4.0.155X5,其
2、诊断阈值为O.36716。对367层图像所做得诊断符合率为77.38%。受试者工作特征曲线(ROC)显示曲线下面积为0.87。结论:本研究表明:脑梗死前期的MⅪ成像(PWI)可有效地检出病例,并对其进行分期。PWI是目前比较理想的功能性磁共振检查方法。DWI不失为一个早期发现脑梗死的检查方法。磁共振血管成像(MRA)为脑灌注成像提供了参考图像。关键词核磁共振;脑梗死:灌注;弥散;回归方程;诊断Astudyondiagnosiscombinedcerebralperfusionwithdiffusionofmagneticresonanceimagingincerebralpre-
