1、苏州大学硕士学位论文颅内动脉瘤的显微手术治疗姓名：鲁峻申请学位级别：硕士专业：神经外科指导教师：张世明20070401颅内动脉瘤的显微手术治疗中文提要结果4l例患者均成功夹闭动脉瘤。术后3例出现脑血管痉挛、脑梗死致偏瘫、失语，l例有精神症状。5例有动眼神经麻痹，随访6个月后4例症状消失。l例大脑中动脉瘤术后14天再次破裂出血，再次手术夹闭。随访2--12个月，恢复良好35例，轻残4例，重残2例，无死亡病例。结论头颅CT扫描是诊断自发性SAH首选的检查方法，DSA仍是诊断颅内动脉瘤最有价值的检查方法。早期手术可以避免动脉瘤再次出血，而且可以

2、清除蛛网膜下腔的凝血块和血性脑脊液，减轻或消除脑血管痉挛。晚期手术可能使部分患者因再出血而死亡，失去治疗机会。动脉瘤夹闭手术难度大，技术要求高，需要手术者掌握熟练的显微手术技巧。显微外科技术的应用能大大提高颅内动脉瘤的手术成功率。Hunt分级Ⅲ级及Ⅲ级以内者手术后效果较为显著，Ⅳ级及以上者术后致残率明显增高。术后积极加强并发症的预防和治疗，能减低患者的致残、致死率，提高生活质量。关键词颅内动脉瘤显微外科手术蛛网膜下腔出血电子计算机X线断层扫描核磁共振数字减影血管造影血管痉挛Hunt-Hess分级迟发性神经功能障碍·作者：鲁峻指导教师：张世
