2、析赤峰地区252例实施支架置入术的脑供血血管狭窄患者病历,将252例患者依据支架置人部位分为颅内和颅外两组,分析两组病例血管狭窄的好发部位,比较两组病例手术成功率、并发症发生率、再狭窄率及近期预后情况。结果1.颅内组53例,占总病例21.03%;颅外组199例,占总病例78.97%。颅外组颈内动脉起始段狭窄者84例、椎动脉起始段狭窄者95例,分别占颅外组的42.21%和47.74%;颅内组椎动脉颅内段狭窄者22例、基底动脉狭窄者20例,分别占颅内组的41.50%和37.73%。2.颅内组并发症的发生率为32.08%;
3、颅外组为2.51%,两组并发症的发生率有显著性差异(P<0.05)o3.术后一年颅内组发生脑梗死1例、TIA2例,发生率为1.89%和3.78%;颅外组发生脑卒中1例,发生率为0.50%,无TIA病例,两组发生率有显著差异(P(0.01)。结论1.脑供血血管狭窄颅外血管发生率高于颅内血管;2.颈内动脉和椎动脉起始段为颅外脑供血血管狭窄的好发部位,椎动脉颅内段和基底动脉为颅内血管狭窄的好发部位;3.颅内血管支架置人操作较颅外复杂,操作风险大,并发症发生率高;4.颅内血管支架术术后再狭窄率及近期发生脑梗死和TIA的比例较
