3、内动脉瘤的病理组织学改变,以及CHRM5在颅内动脉瘤发生、发展和破裂机制中所起的作用。方法方法:收集泸州医学院附属医院神经外科于2010年5月~2012年2月经开颅手术,术中夹闭囊性动脉瘤瘤颈后切除瘤囊所得的40例临床标本;所有动脉瘤标本均在保证患者生命和手术安全的情况下取得,其中男13例,女27例;15例对照组标本为正常动脉,包括大脑皮质动脉7例,大脑前动脉分支4例,大脑中动脉分支4例,标本均取自泸州医学院附属医院神经外科同期颅脑外伤患者行颅内血肿清除术或内减压术的病人;对所有标本进行了肉眼大体观察及光镜检查;制备切片并应用免疫组化SP法检测CHRM5在两组标本中的表达差异性。结果:1.肉
4、眼大体观察:肉眼观察病例组动脉瘤体标本多呈紫红色、暗蓝色或间有棕褐色,部分可见动脉粥样硬化斑块;部分动脉瘤瘤腔内有血栓附着,血栓呈分层洋葱状,瘤壁厚薄不均,动脉瘤瘤顶部明显变薄,有的仅剩一层膜状组织,部分病例可见破裂口。2.常规HE染色组织学观察:颅内囊性动脉瘤瘤壁的主要改变为:内皮细胞明显减少,甚至消失,内弹力层严重损伤,中膜血管平滑肌细胞(VSMC)数量减少,被大量的纤维组织填充取代,外膜胶原纤维排列疏松,有炎性细胞弥漫性浸润,部分动脉瘤瘤壁外膜内毛细血管增生。部分瘤壁上可观察到钙盐颗粒的沉积,血栓形成,以及各阶段动脉粥样硬化的病理改变。而正常动脉壁平滑肌细胞排列规则,各层结构正常,管壁
5、无炎性细胞浸润,内皮细胞无缺损,内弹力层完整,均匀一致。3.免疫组化染色:CHRM5在动脉瘤壁和正常动脉的内膜、中膜及外膜均有表达,阳性细胞为内皮细胞、平滑肌细胞和巨噬细胞,表达部位一细胞核、细胞浆呈棕黄色或与苏木素复染后的蓝色重合的棕色为特征。与对照组动脉壁比较,CHRM5在动脉瘤壁呈低表达,差异有统计学意义,P<0.01。但是CHRM5在动3泸州医学院硕士学位论文脉瘤壁和正常动脉的内膜、中膜及外膜的阳性表达差异无统计学意义。结论:1.颅内动脉瘤瘤壁的主要病理学改变是血管壁的炎症反应和血管基质结构的严重损伤以及动脉粥样硬化,参与炎症反应的细胞主要是淋巴细胞和巨噬细胞;2CHRM5在人脑囊性
