1、一对同卵双胞胎同患急性淋巴细胞白血病并文献复习Wfi熊方建培SW中山大学孙逸仙纪念医院儿科摘要：目的分析罕见的同卵双胞胎同患急性淋巴细胞白血病(ALL)病例，探讨白血病发病原因。方法分析1对同患ALL的同卵双胞胎病例的发病、确诊及治疗过程，并以“同卵双(胞)胎”及“急性淋巴细胞白血病”或“monozygotictwins”及“acutelymphoblasticleukemia”为检索词，对以下数据库的相关论文进行检索:PubMed、中国生物医学文献服务系统、中国期刊全文数据库、万方数据知识服务平台、维普屮文科技期刊数据库。收集并分析检索到的相关病例的资料。结果该对同卵双胞胎患儿先后患上A

2、LL,其白血病融合基因均未见笄常，但其染色体异常不一致，临床分型也不一致，分别被定为ALL的高危型与中危型，经过初次诱导缓解治疗后第33日骨髓均达到完全缓解，且微小残留<0.01%，随访均未见骨髓复发及髓外复发。检索文献收集到5对冋卵双胞胎ALL病例，均有基因或染色体核型异常，其屮有2例骨髓复发死亡，余下8例随访显示未复发。结论同卵双胞胎白血病的胎盘内转移假说有一定的合理性，在胎儿造血细胞形成期产生的染色体异常可能是白血病的起始原因，但白血病的发生发展可能需第二次打击事件。关键词：同卵双胞胎;Q血病;胎盘内转移假说;二次打击;作者简介：周敦华，E-mail:zdunhua@163.com收
