1、尿毒症患者心脏病变的超声观察及相关因素分析【摘要】目的探讨心脏超声在评价尿毒症患者心脏结构、功能方面病理改变的价值。方法运用常规多普勒超声测定120例尿毒症患者的主动脉(A0)、左心房(LA)、左心室(LV)、室间隔厚度(IVST)、左室后壁厚度(PWT)、左室射血分数(EF)、左室短轴缩短率(FS)、每搏量(SV)、二尖瓣口峰值速度比率(E/A)。运用心肌背向散射参数测定心肌背向散射积分(IBS)、心肌背向散射积分周期变化幅度(CVIB)和CVIB/IBS比值等参数,并与100例正常体检者对照,综合病程、病史、病理改变综合分析
2、。结果尿毒症患者约80.0%左心房扩大、约66.7%左心室扩大,约65.0%室间隔增厚、约76.6%左室后壁增厚,约85.0%左室重量指数(LVMI)超重,A峰升高、E/A2者共约占75.0%,VE/VA、CVIB/IBS均不同程度减低。心肌回声不均匀增强或弥漫性增强、同时伴有心包积液、瓣膜增厚钙化、返流等病理变化。结论心脏超声可做为尿毒症患者常规检查项目,尤其是心肌背向散射积分测定的联合应用,不仅可及时检出尿毒症心脏病变,提示患者预后,而且可指导临床治疗。【关键词】尿毒症;心脏病变;超声观察DOI:10.14163/j.cnk
