1、INCIDENTINVESTIGATIONFORM事件调查表CompleteforallMedicalAidInjuries,recordableinjuries(seeScheduleIforexplanation)andanyotherincidentswithseriouspotentialforinjuryordamageinthepresenceofthepersonnelindicated.UseScheduleIItohelpidentifythecausesoftheincident.UseScheduleIIItohelpidentifythefollowuprequired
