1、多层螺旋CT对婴幼儿先天性感音性耳聋的诊断价值作者:边致强,刘亚群,李笃民,崔凤玉作者单位:山东大学齐鲁医院影像中心,济南250012【摘要】冃的探讨多层螺旋CT(multislicecomputedtomography,MSCT)对婴幼儿先天性感音性耳聋(sensorineuralhearingloss,SNHL)的诊断价值。方法对74例婴幼儿SNHL患者采用颖骨轴位高分辨率MSCT扫描及多平面重建(MPR),对感兴趣内耳用容积再现技术(VRT)进行重建。结果63例语前聋组患者中42例(84耳)双侧内耳MSCT表现无明显异常;21例(42耳)显示内耳骨迷路畸形(其中2
2、9耳多种畸形共存):耳蜗间隔发育不全I型1例(2耳),耳蜗间隔发育不全II型(Mondini畸形)4例(8耳),前庭■外半规管畸形12例(24耳),前庭导水管扩大13例(26耳);内耳道畸形3例(3耳);外耳道未发育1例(1耳);其中37耳MSCT横断面图像和MPR,VRT图像均可清晰显示畸形的部位和程度,5耳Mondini型畸形患者VRT图像较MSCT横断面图像更能清晰的显示畸形部位;7例(12耳)分泌性中耳炎。11例语后聋组中显示8例(16耳)内耳结构基本正常,3例(6耳)为大前庭水管综合征,3例(6耳)分泌性中耳炎。结论MSCT结合MPR及VRT可更加准确的对内耳

6、ears)hadinternalauditorycanalabnormality,and1(1ear)hadbonyexternalauditorycanalabsence・Ofthemalformations,alltheaxial,MPRandVRTimagescoulddisplaythesiteanddegreein37ears.VRTimagesweresuperiortotheaxialimagesindisplayingthemalformationsin5earswithMondinimalformationand12earswithsecretoryo
7、titismedia.Ofthe11patientswithpostspeechdeafness,8patients(16ears)wereprovedtobenormal,and3patients(6ears)hadlargevestibularaqueductsyndrome,3patients(6ears)hadsecretoryotitismedia.ConclusionMSCTallowsacomprehensiveassessmentofvariouscongenitalearmalformationsbyhighqualit
