1、提要目的：观察肱骨近端内固定锁定系统（proximalhumerusinternallockingsystem：PHILOS）治疗肱骨近端Neer三部分和四部分骨折的临床疗效。方法：收集肱骨近端Neer三部分和四部分骨折的病例共40例，其中男性11例，女性29例，平均年龄为57.5岁(19岁一87岁)，左侧为20例，右侧为20例。根据Neer分型标准，三部分骨折为26例，四部分骨折为14例。按Constant-Murley绝对值评分回顾性分析比较采用PHILOS在肱骨近端三部分和四部分骨折的疗效。Constant-Murley绝对值评分法如下：满分为100分，

2、疼痛为15分，日常生活活动为20分，三角肌力量为25分，主要活动范围40分。术后总评分90分以上为优，80~89分为良，70~79分为可，70分以下为差，优、良总和所占的百分比为优良率。结果：所有40例患者均术后随访。采用Constant-Murley绝对值评分，肱骨近端内固定锁定系统(PHILOS)治疗满意率为90%。结论：肱骨近端内固定锁定系统（PHILOS）对于复杂的、粉碎的肱骨近端骨是一种较好的内固定物，切开复位PHILOS治疗后早期功能锻炼是决定疗效的关键。此种治疗方法值得临床应用及进一步的研究。关键词肱骨近端骨折；肱骨近端内固定锁定系统；切开复位内
