1、医疗耗材采购合同篇一：医用耗材购销合同医用耗材购销合同合同编号:2014-甲方：乙方：为了进一步加强医用耗材购销管理，严格执行采购程序，规范供销渠道，医院根据临床需要通过公开招标或议价的形式，向乙方采购所需产品。为确保产品质量，保证临床医疗安全，甲、乙双方根据国家相关法律、法规，在平等、互信、互利、互惠的基础上，一致达成如下供货协议，以资共同遵守：一、供货范围、品名及价格等乙方为甲方提供各类耗材共计种，所供医用耗材具体品名、规格型号、单位、单价、厂家等以双方集中招议标结果为准,并附清单。计划外产品采购以双方临时议价为准。二、交货时间、地点与方式
